Get Involved forms collect only the information a visitor chooses to submit for the stated inquiry. Until a secure form endpoint is configured, the forms do not transmit information and display a not-connected message. When configured, the selected form provider’s privacy and security terms will also apply.

Information submitted through a connected form or shared by email is used to review the inquiry and coordinate appropriate follow-up. Do not submit Social Security numbers, detailed medical records, account credentials, or unrelated sensitive information through this public website.

Website information is provided for general informational purposes. Services, eligibility, schedules, locations and pricing remain subject to direct confirmation by Elite NXT Level.

Sessions and consultations are by appointment. Submitting an email or form request does not create an appointment, enrollment, employment, contractor, volunteer, vendor or partnership relationship, or guarantee service. Participation may require separate agreements, waivers, facility access and applicable fees.

Separate waivers may be required for personal training, wrestling, martial arts, sports participation, competitions and wellness services. Programs marked Coming Soon or In Development do not accept payment or enrollment unless specifically announced.

Clients must provide at least 24 hours’ notice to cancel or reschedule a scheduled session.

Cancellations or rescheduling requests made with less than 24 hours’ notice will be charged a $35 late-cancellation fee.

A client who fails to attend a scheduled appointment without proper notice will be charged the full price of the scheduled session.

Programs, services, schedules, locations, instructors, coaches, pricing and eligibility may change. Programs marked Coming Soon or In Development are planned services and are not represented as currently available. No competition or tournament registration is active until a specific event date, venue, rules, divisions, safety standards and registration process are officially announced.

Appointments are not confirmed until the client receives direct booking confirmation from Elite NXT Level. Pricing, availability, and location are subject to confirmation at the time of booking.

Nutrition and wellness services are educational and supportive. They do not provide medical nutrition therapy, prescription diets, diagnosis, treatment, or medical care.

Elite NXT Level Wellness does not diagnose, treat, cure, prescribe or replace medical, mental-health, rehabilitation or other licensed healthcare services. Clients should work with qualified licensed healthcare professionals when appropriate.

New clients may be required to complete appropriate intake forms, health/history questionnaires, participation agreements, waivers and other documents before beginning services. Certain activities may require additional forms or medical clearance when appropriate.

Detailed private health questions are not collected on this public homepage. Participants should consult a qualified healthcare professional before starting an activity when medical guidance or clearance is appropriate.
